I checked the ABO medal roll and found the following:
There are three 'D J van der Heever' (note 'der', not 'den').
There are two 'M J Joubert'.
Stefanus Engelbertus Jansen van Rensburg, Middelburg Commando.
Jan Benjamin Homan, General Liebenberg's Commando.

I have not seen an original of the ABO roll, but I have seen several photocopies. Mine is a copy of a copy of a copy .......

Perhaps it would be worth trying AbeBooks, or similar site. The title page has it as:
Die Medaljerol/ The Medal Roll. Dekoratie voor Trouwe Dienst/Die Zuidafrikaanse Republiek en Oranje Vrystaat Oorlogsmedalje/Lint voor Verwonding.
