My grandmother related to my aunt in 1964 that my grandfather, Thomas Spencer Slater b. 1871 returned to England from America after immigrating in 1886 and served in the 5th Irish Lancers from 15 March 1983 to 14 May 1901. They were married and returned to America the same year. A T Slater is listed under Medals and awards as having QSA(3). No. 4215 and invalided out of service. This fits my grandmothers description. Is it possible to acquire more information with the No. 4215?
I hope to avail myself of the information presented and if I can be of any assistance I will gladly help.

I do not know what you mean. But, The Thomas Slater I was looking for was not Service No. 4215 it is 1721. I found the original Militia Attestation into the 3rd Royal Lancaster Regiment of Thomas Slater in 1888. The dates, places and names correspond to what was provided by my Grandmother.
