That must indeed be a unique combination! I hope that someone is able to reproduce the pages here, and thank you for giving permission for this to be done.

Coincidentally, I have a 6-clasp SALH QSA to a Canadian, who claimed earlier service in the 1898 Spanish-American War, which would have entitled him to the US Army Service Medal for that campaign. Unfortunately, I could not confirm it.
