This memorial is in the churchyard of St John the Evangelist's Church, Bellerby, North Yorkshire, 1½ miles north of Leyburn. There are no other inscriptions, so it doesn't seem to have also been used as the family grave.

Herbert William Thistlethwaite is in the 1891 census returns as being aged 13, the eldest of two sons of Edmund and Mary Eleanor Thistlethwaite, and born at Bellerby. The family lived at Bellerby Moor Road.
