In St Lawrence's Church, Frodingham, Scunthorpe. The plaque's not looking to be in very good condition along the top and bottom edges. Possibly water damage?
I looked in the local paper, the Lindsey and Lincolnshire Echo, but couldn't find a report of his death in the weeks following.
