If you have his KSA and the last set of rivets on the QSA are official, then it is a rare mistake and most interesting, however, If you bought the medal on it's own without the KSA and the last rivets are not official, you can not unreasonably believe the South Africa 1901 clasp has been added at some point, perhaps just to deceive.

Yes, the SA01 clasp on the QSA should not be there. The date clasps (SA01 and SA02) are only ever on the QSA or KSA. His correct entitlement is QSA (6) and KSA (2).

I don't think it affects the value of the medals. It is not the same situation where people added clasps to their medals to which they were not entitled.

I would imagine the addition was made many years ago so I would leave it as it is and we can continue to ponder how it came about.

Well, you have my thoughts, if the rivets are not official, then it's just been added, at some point, five and six clasp medals are common, once you get to seven and eight, the medals start to become, not only harder to find, but, rather expensive too, in the old days, it was much easier for fraud to take place, these days, the rolls and of course the all important verification are instant.
