From memory, that combination of bars on a C.I.V. QSA medal appears to have been quite normal; memory confirmed by a glance at WO100/231. For example, my "1541 Pte. J. McFadden, C.I.V." with clasps CC/OFS/ Joh/DH.

'The most interesting object in the Guildhall just now is the Vierkleur which the CIV captured at Jacobsdal in March last. It is placed between the flags of the Royal Fusiliers (London Regiment) which Colonel Guyon left in the charge of the City on January 6 before leaving for the front.'
