Yes Christelle. The entry here means that he applied for his medal. It should then have been issued to him.

Yes, there are the forms each applicant completed which are housed at the archives in Pretoria. If you contact a research such as Audrey Portman (details in contacts above) she will be able to obtain copies for you (at a cost).
