Andries Christoffel van Schalkwyk completed his Vorm 'B' and it was approved on 26th February 1938. He served under the command of Generaal Beyers, Kaptein du Toit, Kommandant Celliers & Veldkornet Viljoen, which was interpreted by the Union Defense Force as having served as part of the Western Transvaal Commando's (as he had not completed the portion of the Vorm 'B' for the unit in which he served)

Andries Christoffel Martinus Jacobus van Schalkwyk was born on February 16th 1876, he adopted the shortened form of his forenames (Andries Christoffel) and married Aletta Johanna Catharina De Nysschen in Pietersberg, ZAR on 28 December 1898.

His Vorm 'B' is not accurate as he surrendered to the British Authorities at the Pietersburg R.C together with his wife & two infant sons on 8th May 1901 (possession of 2 cows & a calf recognized). He absconded on 23 January 1902 and returned to his Commando, serving out the remainder of the war until he returned on the 27th June 1902 to reunite with his family and take them home to his farm 'Rondebosch'

What remained of his family - in the Pietersbug concentration camp, his children had perished.

Dirk Cornelius Johannes van Schalkwyk, aged 2 years, died of "malaria & convulsions".
